Annette Formicola (Phillips) Passed away January 5, 2012 at age 85. Predeceased by her husband, Anthony, grandson, Jeremy; and siblings, Irving, Solomon Emanuel, Maurice, Joseph, Cecelia & Leila. She is survived by her loving children, Kathy (William) Breen, Ellen (Greg) VanEtten & Thomas (Lee Ann) Formicola; grandchildren, Rebecca, William, Jessica, Greg, Nolan, Tom & Kira; great grandsons, Joshua & Patrick; brothers, David (Florence) & Milton Phillips; nieces & nephews.
